amendments to Rule Sections 12E-1.004 and 12E-1.006, Florida 
Administrative Code.  (Attachment 2)

		12E-1.004	Application for Services, Application Forms and Fee
		12E-1.006	Request for Reconsideration

		The proposed rule amendment of Rule Section 12E-1.006, F.A.C.,  adjusts time frames and clarifies procedures for requests for reconsideration.  Rule Section 12E-1.004, F.A.C., amends form titles and six revised forms are adopted.
