ALL DEPENDENTS MUST BE CERTIFIED IF YOU ARE MAKING A QUALIFYING STATUS CHANGE (QSC) EVENT CHANGE

The preferred, easiest and fastest way to make changes is via the People First website. If you choose to use a form instead to make an allowable QSC event, you must certify that ALL dependents you are covering are eligible (including those currently covered under ANY state insurance plan) before making an election. You may be liable for any and all premiums and claims paid for any dependent deemed ineligible.

• Complete each election form, including the dependent eligibility certification section, in its entirety and mail or fax to People First.
