Life InsuranceThe State of Florida offers eligible full-time and part-time employees term life insurance, including an accidental death & dismemberment (AD&D) benefit through Minnesota Life Insurance Company. Basic Term Life Insurance Coverage and PremiumsBenefits and premium costs depend on your classification:
AD&DBasic life insurance coverage includes AD&D coverage. In the event of accidental death or dismemberment, payment will be made based on the injury's severity - equal to between 25% and 100% of the coverage amount. Optional CoverageState employees who enroll in basic term life insurance can also buy additional term life insurance - from 1 to 5 times base annual earnings, up to a $500,000 benefit. Medical underwriting may be required. You pay the entire cost of optional coverage. You Name Your BeneficiaryDeath benefits will be paid to your named beneficiary(ies). You need to submit a new beneficiary designation form. Accelerated Death BenefitBoth the basic and optional life programs include an advanced benefit if a covered member receives a terminal illness diagnosis that anticipates death within a one-year period. You may be eligible for an advanced payment of up to 100% of your combined basic/optional life insurance benefits.
