The State of Florida offers term life insurance to eligible full-time and part-time employees.

Basic Term Life Insurance Coverage and Premiums

A free Basic Life benefit of $25,000 is available to all full-time employees (part-time employees pay prorated premiums based on their FTE). Enrollment for full-time new hires is automatic.

Accidental Death & Dismemberment (AD&D)

Basic life insurance coverage includes AD&D coverage. In the event of accidental death or dismemberment, payment will be made based on the injury's severity - equal to between 25 percent and 100 percent of the coverage amount.

Optional Coverage

State employees who enroll in basic term life insurance can also buy additional term life insurance - from 1 to 7 times base annual earnings, up to a maximum optional coverage benefit of $1,000,000. You Name Your Beneficiary

To designate or change your beneficiary online, go to www.lifebenefits.com/florida, or get a form at Forms and Publications and send it to Minnesota Life’s Tallahassee branch office at the address on the form. If you do not designate a beneficiary in writing, Minnesota Life pays the proceeds according to the default beneficiary provisions of the policy in this order: your spouse, children, parents and the personal representative of your estate. Payments made to an estate, however, may result in a reduction in total benefits due to taxes and probate costs.

Accelerated Death Benefit

Both the basic and optional life programs include an advanced benefit if a covered member receives a terminal illness diagnosis that anticipates death within a one-year period. You may be eligible for an advanced payment of up to 100 percent of your combined basic/optional life insurance benefits.
