This addendum and the listed attachments forms a part of the contract documents and modifies the original bidding documents. Acknowledgement of this addendum in Exhibit 4 - Bid Proposal form is required. Concerns with this addendum should be addressed to: ng.fl.flarng.list.cfmo-contracting@mail.mil Answers to Questions (Q) / Clarifications (C) / Modifications (M) Q1. There are two alternates that we see listed within the plans; one for Concrete Paving across the Box culverts, and the other to provide Guardrail on either side at all box culvert locations. The Bid forms (Exhibits 4&5) do not have a specific place for these alternates. There is a line item for the Base bid as a lump sum proposal, and then the following is listed: “With foregoing as a Base Bid, the following costs of alternate proposals are submitted in accordance with the drawings and specifications.” Are the alternates to be priced? If so, are we to write in the 2 alternates under this Item? Or will the bid form be revised to provide a location to reflect the price of these alternates. A1. Bidding drawings have been revised to indicate locations of Alternate Bid Items: ABI #001, W-Beam Guide Railing and ABI #002, Concrete Paving at Box Culvert as applicable. Refer to revised drawings issued as part of this RFI Reply and/or Addendum for additional information. Bidders must use the revised Exhibits 4 and 5 attached below. Does not apply to project 212074. Q2. We would like to request info on the size, graduation and type of rip rap to be used on the box culvert projects. A2. Rip-rap shall comply with FDOT Standards, Section 530 – Revetment Systems. Rip-rap size and/or gradation shall vary between 8” to 18”, typical. Does not apply to project 212074. Q3. What size Bedding Stone shall be used where called out under the Box culverts and Headwalls? A3. FDOT Standard 57 stone shall be used in these locations. Does not apply to project 212074. Q4. Can a specification and or manufacture be provided for the concrete waffles called out? We cannot find any information in regards to this item and feel there is not enough information to accurately price it. Only applies to project 212074. A4. Please see Addendum #3, issued 5/14/2020, for information requested. Only applies to project 212074. Q5. Are permits required? A5. No permits are not required. Applies to all projects. Q6. Is Builder’s Risk Insurance required? A6. No, Builder’s Risk Insurance is not required. Applies to all projects. Q7. Will we be allowed to burn the tree debris after removing onsite or anywhere else on Camp Blanding? A7. NO, burning is not permitted on CBJTC. Applies to all projects. Q8. Was the Pre-Bid meeting mandatory? A8. Attendance at the Pre-Bid meeting was NOT mandatory but highly encouraged. Applies to all projects. Q9. Are there any specs for the concrete waffles to be used on the tank trail? A9. Please see Addendum #3, issued 5/14/2020, for information requested. Only applies to project 212074. Attachment List 1.
