Addendum #001 includes revisions to Section 2.6.3.2, Program Discharge and Section 2.11, Staff Background/Criminal Record Checks. Addendum #001 also includes the addition of Attachment XIV, FDC Procedure 208.013. ------------------------------------------------------------------------------- The Department is requesting Proposals from qualified, responsive, and responsible Vendors, to provide residential substance use disorder treatment and behavioral health services, utilizing the Therapeutic Model, to Offenders in all, or a combination of, Clay County (Judicial Circuit 4), Alachua County (Judicial Circuit 8) and Bradford County (Judicial Circuit 8). Proposals will be opened at the address below at 2:00 P.M., Eastern Time (ET), on September 12, 2019.
